Rob Flint, a PhD student at the London College of Printing, London Institute (http://www.research.linst.ac.uk) has written one of the most thorough and erudite documents on the topic. He can be reached via the LINST research office.
Michael Heap has also been writing a brief survey on the topic. And you can find him here on the boards.
You might also consider Woody Vasulka's superb Eigenwelt der Apparate-Welt, published by Ars Electronica (http://www.aec.at) in 1992. Although not about VJing in any sense, it is one of the preeminent books on the history of media and video art.

** On topic - It's also worth mentioning, that Stan Brakhage died last week - one of the pioneers of Lyrical/Experimental film, worth checking out some of his early works (Mothlight etc..) **

Very on topic, and a sad time for anyone in media art. For those who don't know Brakhage, you'll find his work documented in the books of UK materialist filmmaker and historian Malcolm LeGrice.

Anyone who wants to use me as a resource for these kind of projects is welcome to contact me...
As some of you are aware I have a huge amount of resources covering VJing from 1978 to the present and have a short `VJ history' documentary `Movement of Light' about to make the rounds...
The story of the early days especially is starting to get lost in the mists of time, let's keep the heritage of our art form alive!
I have been going over all that old material while prepping for my 25th anniversary exhibits, so it is all fresh in my mind at the moment & it strikes me that there are many VJ techniques etc from the `Golden' & `Boom' eras that are just waiting to be rediscovered!
I hung on to all those boxes full of stuff for exactly this reason!

Rob Flint, Film, Video and Digitality - an Analysis of Technology and Cultural form in Time-Based Media
Kriel: I have found the title of his paper, but (where) can I contact him?
If his paper is interesting, he should post it to VJforums.
